PTA Check Request Form
This form must be submitted to receive reimbursement from the PTA. Forms must be received by June 14 to receive reimbursement.  There are two ways to submit the form:
 
1) The form may be submitted electronically by clicking here.
 
2) A paper copy can be viewed/printed by clicking on the link below.  
 
Send the form, plus receipts/invoices associated with the request to Charity Coates (charity.coates@comcast.net) or drop off in the PTA or PTA Treasurer's mail box at the Lower or Upper Campus.
