The Annual Health Information form is requested yearly of parents/guardians of NEW students or incoming kindergartners to let health office staff know about health issues that might interfere with learning or which may result in an emergency during the school day.  If your student has a change in his/her health condition, please fill out a new form and send in to the health office.  This form is also available in Spanish, Hmong and Somali. 

Checkups are strongly recommended for students entering kindergarten and 7th grade. Students who want to play sports in middle and high school must have a physical examination on record (completed within 3 years) with the coach before they can play.  Click on the appropriate form below for your student.
