Signing up to become a PTA member is easy:

2. Drop off the printed form at the school office.
 
3. If you prefer, you can also mail the completed form to LHCS PTA Membership, 4912 Vincent Ave S, Minneapolis, MN 55410
 
We will send you your membership card within two weeks of receiving your form.
